Right pulmonary artery obstruction due to anastomotic pseudo-aneurysm 23 years after implantation of an aortic prosthesis.

چکیده
Fig. 1. Preoperative images. (a) Chest X-ray. (b) Pulmonary scan with Tc-99mMAA shows absence of right lung perfusion. (c) CT image with intravenous contrast shows obstructed right pulmonary artery due to pseudo-aneurysm behind a prosthetic graft that had been implanted 23 years earlier for acute aortic dissection. These findings were accompanied by elevated D-dimer (38.2 mcg dl ) and dyspnoea over a period of 2 years that had rapidly deteriorated over the past month, followed by sudden pain in the right chest. Emergency surgery was thus required to recover right lung perfusion. Clinical and radiological signs of infection were absent.

Surgical Management of Ascending Aortic Aneurysm and Its Complications
Ascending aortic aneurysms involving the proximal aortic arch, arising anywhere from the aortic valve to the innominate artery, represent various problems in which open surgery is generally required. Surgical options include excision of the aortic pathology or wrapping the aneurysm shell with an aortic Dacron graft. Intervention using the latter method can lead to extravasation of blood along t...
متن کاملDissecting aneurysm of the thoracic aorta presenting as right atrial obstruction.
A patient with a dissecting thoracic aortic aneurysm presented with acute low output cardiac failure due to right atrial obstruction. Five years earlier she had had her aortic valve replaced because of aortic regurgitation. Obstruction of the superior vena cava and pulmonary artery has been reported elsewhere as a complication of compression by thoracic aortic dissecting aneurysm. This case was...
متن کاملRight heart failure secondary to compression of the right pulmonary artery by a syphilitic aortic aneurysm.
A 68-year-old man presented with right heart failure due to compression of the right pulmonary artery by a syphilitic aneurysm of the ascending aorta. The diagnosis was made by cardiac catheterization and angiography and it was proven by autopsy. This complication is unusual and supports the experimental evidence that unilateral pulmonary obstruction may be responsible for the development of pu...
متن کاملPercutaneous nephrostomy in an unusual case of ureteric obstruction.
Ureteric obstruction from an iliac artery aneurysm is uncommon.1 We report what we believe to be the first case in which percutaneous nephrostomy was used in the management. CASE REPORT A 70-year-old man presented with rigors. He had a history of severe cardiovascular disease. Examination revealed marked congestive cardiac failure, a palpable abdominal aortic aneurysm and a separate pulsatile m...
متن کاملObstruction of the central pulmonary artery after shunt operations in patients with pulmonary atresia.
The central pulmonary artery was studied angiographically 1-10 years after a palliative operation in 56 patients (mean age 6 years, range 1-20) with congenital heart disease and pulmonary atresia. All of them had originally had a central pulmonary artery that was common to both the right and left pulmonary arteries. Juxtaductal obstruction of the left pulmonary artery had developed in 70% of th...
